Community Outreach
and Wellness Program
The goal of this program is to promote the health and wellness of older adults and their caregivers by providing disease prevention/management information. Most programs are offered at no charge. Volunteers are welcome to assist in the program.
Programs include:
  • Fall and Injury Prevention: A seminar that discusses tips on preventing falls through discussion and participation in games such as bingo, to learn the risks.

  • Medication Management: This program is aimed at teaching older adults how to prevent medication errors and drug interactions.

  • Health Fairs: Health and wellness information is provided at local health fairs

  • Wellness Fairs: A volunteer team does medication screenings, foot screenings, blood pressure and blood glucose screenings.

  • Health Information: Diabetes, arthritis, high blood pressure, and mental health (depression and suicide prevention) information provided to older adults and their caregivers.

  • Osteoporosis Screening: Bone density testing is available on specific dates throughout the year.

  • Chronic Disease Self Management Program: A six weeks class for people to aquire the skills and techniques to self manage their disease...heart disease, diabetes, arthritis, emphysema, HIV/AIDS, etc.
